Why should I choose Community Care Rx?
We provide secure and convenient care for your patients. Using a wide variety of techniques and programs, we can help ensure the accuracy of your patient’s pharmaceutical needs.
We are the first pharmacy to use the Code Amber Alertag which allows the medical provider to access vital information during emergencies, ultimately saving many lives. It can be accessed through any internet-based device. For added protection, it utilizes three layers of protection which must be unlocked to receive access. The levels include a QR Code, a password, and a key code.
To further connect with you we use a program called Webconnect. This program is a secure and safe network that connects our pharmacy and your facility. Using this program you can submit a refill for the patient’s medications for the next or even the same day. This digital system also reduces misplaced refill orders and faxed refills, and you can also view your patient’s summary statements for billing and pricing.
Does Community Care Rx offer vaccination services?
The staff of Community Care Rx is NYC certified to provide on-site immunization for patients who are the age of 18 or older. We can set up vaccine clinics with you to protect your staff and your customers. Community Care Rx offers influenza, pneumonia, meningitis and shingles vaccines, and we will work with you to bill Medicare, Medicaid, and all other private insurance directly. Quite often there’s no cost to the resident! We also offer the vaccine for meningitis.
What types of training does your long term care pharmacy offer?
Community Care RX has developed programs to help increase pharmacy literacy among your facility’s staff through our business affiliation with multiple health care education centers and education experts such as The Beacon Group. Our staff consists of accredited HIV, diabetic, and Asthma educators who provide on-site training for your staff. We also offer continuing education courses, credentialed alcoholism and substance abuse counselor training (CASAC), and Cardiopulmonary Resuscitation Courses (CPR) that can be scheduled at your facility regularly.
When do medication deliveries take place?
Our committed delivery team delivers 7 days a week, 365 days a year, providing you with speedy and reliable service. To ensure the accuracy of each package, every scheduled delivery consists of consolidated delivery sheets that include resident name, the medications being delivered and the quantities of the delivered medication. If any emergencies or medication questions occur after we have closed there is an on-call consultant pharmacist available to answer any questions and dispense emergency medications 24 hours a day, 7 days a week.
What if I have questions after the pharmacy has closed?
At Community Care Rx we offer a consultant pharmacist, who is on call and available to answer any questions and dispense any medications needed for your patients in case of emergency. This service is available 24 hours a day, and 7 days a week.
What is the pill card you offer used for?
A pill card uses a simplified, visual way to show all the medications the particular patient needs to take on a regular basis, at the certain times. The cards use colorful pictures and simple phrases for each medicine to show when and how to take each type of medication. So complete accuracy can be achieved and confirmed with minimum worry.
Do you offer medical supplies? Can I rent them for short term use or purchase them for long care use?
For the convenience of our customers, we offer over 5,000 surgical and medical items. They help to improve the well being of the individuals living with chronic conditions and difficult disabilities. The use of them can be temporary or permanent; we offer the convenience of renting or purchasing the medical item as needed.
Community Care RX also offers equipment repairs to medical and surgical items, and we bill all insurances, including Medicare and Medicaid!
What happens with unused medications?
We can dispose of properly at our long-term care pharmacy through our Safe Medication Destruction Program. This prevents medication errors and prevents any confusion with the nursing staff at your facility. There is no charge for this service and can be done within the same day!
