Community Care Rx can be a vital resource for both patients and their caregivers, thereby improving quality of life. A comprehensive understanding of the etiology, pathophysiology, and stages of Alzeihmer's Disease, Dementia and other memory related diseases, as well as pharmacologic therapy, is imperative to provide effective care to the patient. We can assist these patients with through monitoring drug regimens for potential drug interactions as well as possible contraindications. More importantly we are empathic towards these patients and their caregivers, keeping them informed about new developments in the fight against these conditions and suggesting resources of information for them.

Caring for a patient with memory related diseases involves more than drug treatment. We assist caregivers in joining local support groups and to take care of themselves and seek assistance when warranted.

SERVICE GOALS

MEDICATION REGIMEN REVIEW (MRR)

  • Recommendation of therapeutic substitutions resulting in reduced medication cost to the facility or resident while providing appropriate therapy
  • A collaboration of clinical initiatives with physician and nurses to enhance resident care
  • Detection and flagging of the use of unnecessary, high risk, or Potentially Inappropriate Medications (PIMs) that should be avoided
  • Management of psychotropic medications and their gradual dose reductions
  • Source of pharmaceutical information for physicians, nurses, pharmacy, family members, and residents

MEDICATION MANAGEMENT SERVICES (MMS)

  • Development, implementation, and periodic review of policies and procedures
  • Inspection of stored medications and medication storage areas
  • Controlled substance inventory check or reconciliation
  • Witnessing of unused medication disposal or destruction
  • eMAR implementation

COMMITTEE MEMBER SERVICES

  • Medication costs and evaluations for appropriate use
  • Analyses of reported medication errors
  • Provision of trending or quality assurance reports

TRAINING AND EDUCATIONAL SERVICES

  • Periodic training and monitoring of staff performance
