Transition of Care (TOC)
Our Transition of Care services support safe, accurate medication handoffs when residents move between hospitals, sites, and home.
About Transition of Care (TOC)
Medication discrepancies at care transitions are a leading cause of readmissions. Our pharmacists reconcile medications and coordinate with your team to keep therapy consistent.
Our pharmacists collaborate with the client, caregivers, and the interdisciplinary care team to identify and address medication access barriers, deliver medication-related education and counseling, and provide follow-up support that helps reduce readmissions. We also help your organization identify opportunities to reduce the likelihood of medication-related errors and oversights.
The responsible way to discharge residents
A well-managed discharge reduces readmissions, helps ensure deficiency-free surveys, and improves star ratings. The process is simple: notify Community Care Rx 48 hours prior to discharge and email or fax the discharge papers. Our pharmacists do the rest — including Med to Bed delivery on the day of discharge.
